- Trip Release Form for Adults 21+ (PDF) OR Trip Release Form for Teens (PDF) – notarized
- Emergency Contacts, Allergies & Medication Form (PDF) – signed by a parent
- Photo Permission Form (PDF)
- Under 21 only: Permission to Travel (PDF) – notarized
- Copy of the Picture/Signature pages of your Passport – expiration date February 2026 or later
- Flight details, including flight confirmation code
- Alternative drop-off/pick-up details if you/your child are not traveling with the group
- Payment #2: $275.00 (exact amount to be announced)
